Please complete the following survey right before starting "Core Lesson 1: What are the SSF Guidelines?" with your participant group. This will allow you to establish a baseline from which we can compare students and learners' progress over time. Return to this survey after facilitating "Core Lesson 13: The SSF Guidelines & Me" or "Enrichment Activity: Closing Activity." This assessment tool requires careful consideration. It is essential that the person submitting feedback has worked closely with youth throughout the curriculum. This will give those completing the assessment an opportunity to more thoroughly examine the extent to which youth are involved as leaders. This assessment can be submitted again three months, six months, and one year after the curriculum experience finishes to measure the curriculum's effectiveness over time.
